Patient Responsibilities
- Provide accurate information about one’s health and medical history
- Ask questions when instructions and information is unclear
- Communicate with your doctor when you are unable to follow through with your examination
- Be considerate of the rights of other patients and comply with facility guidelines regarding conduct, which include but are not limited to, smoking, noise, and respect for the property of others
- Understand and accept the consequences that may result from refusing the examination
- Provide accurate insurance information and or work with facility administration to arrange for appropriate and prompt payment for any and all services rendered
Patient Rights
You have the right to:
- Considerate and respectful care.
- To be free from all forms of abuse or harassment.
- To know the names and roles of people treating you.
- To receive information in a manner that is understandable by you.
- To receive compassionate care that respects your personal, spiritual, cultural, and religious values and beliefs.
- Privacy
- To expect that examination records are confidential unless you have given permission to release information or reporting is required or permitted by law.
- To consent or decline to take part in research affecting your care. If you choose not to take part, you will receive the most effective care the facility otherwise provides.
- To consent to or refuse any treatment, as permitted by law.
- Review your medical records and to have the information explained, except when restricted by law.
- To review your bill, have the information explained to you, and get a copy of the bill.
